Referrals to Community Resources
As the caregiver for an older adult, you may not be aware of the many resources available in the community to help older adults. If you are concerned about your loved one's ability to live independently, there may be options for him or her. The discharge planner or social worker can discuss these options with you.
Resources Frequently Needed by Older Adults
- Transportation
- Personal care (bathing, dressing)
- Home care (light housekeeping, shopping)
- In-home meal services
- Support services for caregivers
- Adult medical day services
- Financial resources (Pharmacy Assistance, Medical Assistance, etc.)
The discharge planner can provide you with information needed to contact the local Departments of Aging, Social Services and Health, as these agencies offer many important services to older persons.
It is important to note that an individual must agree to receive services that are offered. Unless there is a risk of abuse, neglect or harm to the individual, he or she can choose not to accept any service offered.
